Description: Better Choices, Better Health® Self-Management program is a 6 week program that meets once a week for 2.5 hours. Each class is led by two trained leaders. The class is attended by those who have a chronic health condition, are supporting someone with a health condition, or want to prevent a chronic health condition. The course is offered at local community sites (Community Centre’s, Senior Centre’s, Churches, etc.) and in Healthcare facilities. It is based on Stanford University’s widely tested Chronic Disease Self-Management program. Participants learn how to be good self-managers by dealing with the physical and emotional challenges of long term health conditions.
